About this template

This Microsoft Excel spread sheet template allows you to keep an accurate record of students attending individual lessons.

There is space for up to 30 student names to be recorded on one A4 sheet.

Against each student name there is space to write comments and at the foot of the attendance sheet there is also space to record any comments, incidents or observations relevant to the class as a whole.

This Form cannot be submitted until the missing fields (labelled below in red) have been filled in
